Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is a community-based program serving adults diagnosed with serious, chronic mental health challenges. The evidence-based ACT model is designed for those who are most at risk for psychiatric hospitalizations and for whom traditional outpatient services have not been successful. The team meets daily to coordinate care and engages with clients multiple times each week. Most of the services take place in the community rather than at Jackson County Mental Health. This intensive treatment model allows us to help clients live their best lives possible by being flexible and creative in how we approach care for each person.

The ACT team is made up of different kinds of providers including:

  • Employment specialist
  • Peers
  • Case managers
  • Therapists
  • Nurse
  • Substance abuse treatment specialist
  • Psychiatrist
