MCHNURSE FAMILY Partnership (NFP)Jackson County's NURSE FAMILY Partnership is a home visiting program that empowers expectant parents to transform their lives and create better futures for themselves and their babies. Becoming a parent can be overwhelming and Nurse Family Partnership can help.  First-time parents and parents with other children, who are pregnant and have not yet delivered, who meet the enrollment criteria and live in Jackson County are eligible for free help from a personal nurse who will come to your home or wherever you would like to meet to offer support and assistance when you need it most through your pregnancy and until your baby is 2 years old. Babies First!Babies First! is a nurse home visiting program for families with babies and young children up to age 5. The goal of Babies First! is to help families make sure that their babies are healthy as they grow and learn.

What can you expect?

Your nurse will help you work towards:

  • Having a healthy pregnancy and a healthy child
  • Becoming the parent you want to be
  • Building a strong network of support for you and your child
  • Assuring that your home is safe and hazard free for your developing child to play and explore
  • Setting goals for your family’s future and finding ways to help you reach them
  • Connecting you with resources to support you on your journey so that obstacles can be faced head on
Your nurse will also be able to provide additional support with:

  • Answering any questions or concerns that may arise during your pregnancy and parenting journey
  • Sharing information about what to expect as your child grows and develops
  • Providing lactation and infant/toddler feeding support
  • Sharing ways to play and interact with your growing child to support and enhance their growth and development

CaCoon (CAre COordinatiON) A statewide public health nurse home visiting program that specializes in supporting families coordinate care for their children and youth with special health needs.  CaCoon home visitors provide services for families anytime from birth up to the age of 21.  They support your child’s transition from youth to adulthood by meeting your child and family where you’re at.

What can you expect?

CaCoon nurses partner with you to:

  • Identify your child's strengths and needs.
  • Assist in identifying problems and finding solutions.
  • Connect with healthcare services and other community supports.
  • Make sure your child’s health team works well together.
  • Help gather information to make the best health-related decisions for your child.
